Position
Summary:
Provide personal care and supportive services to individuals in the community in the privacy of their home.
Qualifications:
HHA/PCA 2 certification in good standing according to NYS regulations.
Must be mature and able to deal effectively with the job and its demands.
Must be respectful of the patient's rights and treat a client and his/her belongings with dignity and respect.
Have a sympathetic attitude towards the care of the sick and disabled.
Job Description:
o Provide health related services as outlined in the plan of care authorized by a physician and established by a registered nurse or licensed therapist under the direction of the Aide Matrix established by the NYS Department of Health.
o Provide assistance with personal hygiene, housekeeping and/or other related supportive tasks to a patient with health care needs in his/her home.
o Immediately report changes in client's condition to supervising professional.
o Complete and submit necessary documentation including charting tasks and observations on android device.
o Attend ongoing in-service programs in a timely manner to maintaincertification.
o All other duties as assigned.
Requirements:
o Ensure the safety of the patient, patient's family and/or the employee.
o Maintain HIPPA.
o Provide one's own transportation to and from assignments, and arrive promptly to all assigned HCR work locations.
o Commit to be available for work and acceptassignments when offered.
o Be able to speak and write in English in order to follow the plan of care, communicate effectively with patient and interdisciplinary team.
The physical demands described below must be met by an employee to successfully perform the essential functions of this job, and to ensure patient safety at all times in the household.

While performing the duties of this job, the employee is required to visually recognize objects within 20 feet, have the ability to speak and hear.
