Nurse Pracititioner (Home Based Primary Care)

The Nurse Practitioner functions autonomously within a defined scope of practice, as well as function in a variety of roles in collaboration with other health care professionals. He/she is a direct care provider in the Home Based Primary Care line of Extended Care whose responsibilities may also include education, research, consultation and administrative roles. The Nurse Practitioner (NP) for Home Based Primary Care functions as an interdisciplinary team member in providing primary care to veterans who are high risk high need with complex, chronic, and disabling diseases for whom routine clinic-based care is not effective. The NP's primary functions include physical assessment, cognitive assessment and depression screens, prescribing medications as well as supplies/durable medical equipment, coordination of care, and diagnostic work-ups, in collaboration with the medical director. As an interdisciplinary team member, the NP is required to make home visits, draw labs peripherally, administer vaccinations, and implant and interpret tests. Other responsibilities include, but are not limited to participating in interdisciplinary team discussions, providing staff in-services, participating on selected committees, attending departmental and organization related meetings, providing coverage for clinical program functions, and supervising selected health care staff. He/she will also be required to complete and review non-VA documents including, but not limited to home health aide certification forms, hospice visiting nurse referrals, physical assessment documents from non-VA agencies, Aid and Attendance and applications to long term care facilities. The NP will be required to incorporate technology into his/her practice. Examples of technology include, but not limited to Clinical Video Telehealth (CVT), Care Coordination Home Telehealth (CCHT) and Secure Messaging. The NP should be able to demonstrate leadership skills, maintain confidentiality of patients, and work independently in the absence of supervision.The NP should also be able to communicate clearly and concisely, both orally and in writing. Duties include, but are not limited to:
Obtains health history from the Veteran and/or caregivers. Performs comprehensive physical assessments and needs assessments. Order and interpret diagnostic tests Order supplies and durable medical equipment. Manages wounds. Formulates diagnoses based on medical, physical, psychosocial status and diagnostic test results. Assesses and evaluates patients' responses to services provided, treatment, and medication. Peripheral specimen collection, administration of vaccinations and skin tests. Based on patients' goals and expectations for care, develops care plan and communicate plan to patient, the caregiver or family, and the interdisciplinary team. Makes appropriate referrals to specialists and community resources. Determines the effectiveness of care plans for documentation of patient outcomes and modifying the plan as necessary to achieve medical and health goals; communicating findings and plan of care to patient, the caregiver or family, and the primary care provider. Completes and maintains written records in a timely fashion and in compliance with VAFLHCS rules and regulations. The Nurse Practitioner provides clinical care in home settings. Experience:
2-3 years of nurse practitioner experience that demonstrates leadership and performance in the application of the nursing process to patient care, organizational processes and/or systems and improving patient care outcomes including NP experience in adult health with emphasis in the home-based primary care specialty area or homecare preferred. OR The completion of coursework equivalent to a nursing degree in a MSN Bridge Program that qualifies for professional nursing registration constitutes the completion of an approved course of study of professional nursing. Nurse I Level I - An Associate Degree (ADN) or Diploma in Nursing, with no additional nursing practice/experience required. Nurse I Level II - An ADN or Diploma in Nursing and approximately 1 year of nursing practice/experience; OR an ADN or Diploma in Nursing and a bachelor's degree in a related field with no additional nursing practice/experience; OR a Bachelor's of Science in Nursing (BSN) with no additional nursing practice/experience. Nurse I Level III - An ADN or Diploma in Nursing and approximately 2-3 years of nursing practice/experience; OR an ADN or Diploma in Nursing and a Bachelor's degree in a related field and approximately 1-2 years of nursing practice/experience; OR a BSN with approximately 1-2 years of nursing practice/experience; OR a Master's degree in nursing (MSN) or related field with a BSN and no additional nursing practice/experience. The check can be for multiple reasons such as signing bonus, supplies, etc. The victim will be instructed to deposit the check and use the money for any of these reasons and then instructed to send the remaining funds to the fraudster. The check will bounce and the victim is left responsible.
