Registered Nurse (Specialty Care)

The RN Care Manager for the Medical Specialty clinics is a registered nurse who assures coordination of care and impacts outcomes for Medical/ Specialty Clinic patients at the Calkins Road Rochester Ambulatory Clinic. He/she coordinates services for Veterans throughout the spectrum of care, provides nursing care of Veterans with care providers and collaborates with external disciplines. The RN will provide care management as assigned to the following clinics:
GI/Endoscopy /Cardiology/Dermatology /Derm Surgery/Podiatry/Eye/ Vascular/Telehealth and Primary Care. This is not an all-inclusive list of potential Specialty Clinics. Responsibilities The RN works closely with the Nurse Manager, provider staff and nursing team to provide a highly successful work environment focused on positive work relationships and outstanding patient-centered care, acting as a role model for all other members of the healthcare team. The RN is a leader in the core team and is responsible for successfully collaborating with services, both internal and external; to facilitate transitioning levels of care in order to effectively meet the Veterans' needs and assure seamless continuity of care and access to services. Assures proper nursing support to the provider team and works within the multidisciplinary team in order to assure the most timely and high quality experiences for our patients. The RN provides patient and family health education with a focus on self-management, prevention, and wellness, based on the patient's goals and with a strong focus on scientific fact and discovery. He/she serves as an advocate for Veterans, is a team player to colleagues, and continues to enhance his/her own and the team's professional growth, development and practice. Advanced clinical knowledge, leadership and mastery of nursing practice within a Specialty Care area of nursing:
Ability to deliver efficient, comprehensive and continuous patient-driven holistic care Ability to actively collaborate & communicate within and across systems to coordinate resources within and external to the VA to provide optimal patient care Responsibility to access and utilize multidisciplinary resources, programs and service, within and external to VA to facilitate transitions of care Triages and applies a collaborative team approach in identifying, analyzing, and resolving patient care problems, and develops care goals for resolving patient care problems. Participates in interdisciplinary meetings with patients, families, and team interactions to facilitate collaboration. Initiates and maintains active involvement in quality improvement activities that result in improved outcomes. The incumbent is expected to take professional responsibility for attending in-service and continuing education activities and maintaining licensure. The incumbent is expected to attend mandated annual review programs; is responsible for maintaining excellent avenues of communication with Veterans, families or significant others, and other Care Managers and/or at geographically separate sites, collaborating with the Nurse Manager in meeting staff and patient educational needs, providing continuity of care throughout the spectrum of services. The incumbent is subject to the provisions set by the Nurse Professional Standards Board in areas of probationary period, promotion and other actions requiring Board approval. The incumbent will focus on identifying high risk populations, assess and assist in arranging patient needs, provide discharge planning, facilitate smooth transitions of care, know roles of team members and delegate the daily work appropriately, and will proactively follow and promote access with non-face-to-face visits when appropriate. Current ACLS preferable. Successful completion of this certification is required to maintain position in Specialty Clinic. The RN will provide care management as assigned to the following clinics:

The Accreditation Commission for Education in Nursing (ACEN) or The Commission on Collegiate Nursing Education (CCNE). In cases of graduates of foreign schools of professional nursing, possession of current, full, active and unrestricted registration will meet the requirement of graduation from an approved school of professional nursing. OR The completion of coursework equivalent to a nursing degree in a MSN Bridge Program that qualifies for professional nursing registration constitutes the completion of an approved course of study of professional nursing. Students should submit the certificate of professional nursing to sit for the NCLEX to the VA along with a copy of the MSN transcript. (Reference VA Handbook 5005, Appendix G6) OR In cases of graduates of foreign schools of professional nursing, possession of a current, full, active and unrestricted registration will meet the requirement for graduation from an approved school of professional nursing. The following criteria must be met in determining the grade assignment of candidates, and if appropriate, the level within a grade:
Nurse I Level I - An Associate Degree (ADN) or Diploma in Nursing, with no additional nursing practice/experience required. Nurse I Level II - An ADN or Diploma in Nursing and approximately 1 year of nursing practice/experience; OR an ADN or Diploma in Nursing and a bachelor's degree in a related field with no additional nursing practice/experience; OR a Bachelor's of Science in Nursing (BSN) with no additional nursing practice/experience. Nurse I Level III - An ADN or Diploma in Nursing and approximately 2-3 years of nursing practice/experience; OR an ADN or Diploma in Nursing and a Bachelor's degree in a related field and approximately 1-2 years of nursing practice/experience; OR a BSN with approximately 1-2 years of nursing practice/experience; OR a Master's degree in nursing (MSN) or related field with a BSN and no additional nursing practice/experience. This position requires visual acuity, keen hearing, clear distinctive speech, and manual dexterity. This position requires potentially long periods of continued walking, standing, stooping, sitting, bending, pulling, and pushing. Transferring patients and objects may be required. Mechanical patient lifts are provided. The incumbent may be exposed to infected patients and contaminated materials and may be required to don protective clothing in isolation situations or operative/invasive procedures. The incumbent may occasionally be exposed to patients who are combative secondary to delirium, dementia, or psychiatric disorders. The incumbent must be a mature, flexible, sensible individual capable of working effectively in stressful situations, able to shift priorities based on patient needs. The fraudster will send a check to the victim who has accepted a job. The check can be for multiple reasons such as signing bonus, supplies, etc. The victim will be instructed to deposit the check and use the money for any of these reasons and then instructed to send the remaining funds to the fraudster. The check will bounce and the victim is left responsible.
