Social Worker - HUD-VASH

The social worker is responsible for providing case management for a caseload of Veterans in the HUD-VASH program. The social worker is responsible for reviewing agreements between Section 8, landlords, Public Housing Authorities and the VA in establishing appropriate referral sites for HUD-VASH placements. Performs site visits of HUD-VASH apartments to ensure that Veterans reside in safe environments in compliance with local housing codes. Verifies the Veteran's status, eligibility for VA medical care, and eligibility of program participation. Responsible for ensuring that homeless Veterans are considered as primary participants for program acceptance. Responsible for providing HUD-VASH case management. Case management is defined as the provision of services by VA clinical staff to homeless Veterans and includes all activities to provide appropriate treatment and maintain Veterans in HUD approved stable housing. Coordinates care including serving as an advocate on behalf of the Veterans to ensure comprehensive service delivery, and linking and referring Veterans to the VA Medical Center, VA Regional Offices and/or community agencies. Complies with criminal conflict of interest laws and Executive Branch Standards of conduct so as to avoid conflicts of interest while serving as the HUD-VASH Case Manager. Actively participates in the HUD-VASH Program conference calls and attends training offered by the HUD-VASH. Responsible for following local policy and SOP for the HUD-VASH Program. This includes following policies and documentation requirements as required for CARF and Joint Commission accreditation. Actively participates in program planning and development, advising the supervisor of shifting trends with caseloads and identifying the need for and making recommendations on program procedure or policy changes based upon work experiences. Takes responsibility for improving professional skills, participating in planning and executing specific staff development projects, attending seminars and institutes, and participating in training programs of other disciplines as requested. Performs accountability functions:
1) document all contacts with patients/families in the medical charts; 2) completes statistical and other reports in accordance with VA Medical Center and respective professional discipline service policies. Works with VA Medical Center multi-disciplinary teams and community agencies to strive for developing more effective and efficient ways of delivering healthcare services to HUD-VASH Program Veterans. provides comprehensive discipline-specific services to Veterans and their families. Responsibilities include the management, coordination and provision of discipline-specific services to Veteran and their families in inpatient, outpatient and community settings. In the provision of care in patients from the adult (less than 65 years of age) to the geriatric (greater than 65 years of age), the employee recognizes the age related needs and demonstrates knowledge and care appropriate to that age group. Conduct psychosocial assessments to determine needs of Veterans and family members. Is an active participant in the treatment planning process with other disciplines. Participates in discharge planning with other disciplines and will be responsible for ensuring that discharge plans are executed in a manner that is timely and appropriate. Provides individual, family and group counseling as appropriate. Provides consultation/education to Veterans and families on community resources, advance directives and VA benefits. Acts as a consultant to other team/staff members. Acts as liaison between VA and community resources, to include marketing and public relation duties as needed. Provides case management to Veterans and families, as needed, throughout the continuum of care. Provide a full range of discipline-specific services within commonly accepted standards of professional practice which includes case work, individual, family and group counseling, discharge planning, case management and other services as appropriate. Addresses Advance Directives in accordance with the standards of their work area. Provides wellness/prevention education and facilitate Patient/family support groups as appropriate. Coordinates treatment and communicate with the care lines to facilitate continuity of care for Veterans. Work Schedule:

Have a master's degree in social work (MSW) from a school of social work fully accredited by the Council on Social Work Education (CSWE). Licensure:
Must be licensed by a state at the advanced practice social work with a master's degree in social work. English Language Proficiency:
Social workers must be proficient in spoken and written English in accordance with VA Handbook 5005, Part II, chapter 3, section A, paragraph 3j. Experience:
Completion of a minimum of 2 years of post-MSW degree clinical experience in the field of health care social work (VA or non-VA experience) and licensure or certification in a state at the independent practice level. Grade Determinations:
GS-9 Social Worker GS-9 is the entry level grade for the GS-185 social work series and is used for social workers with less than one year of post-MSW experience and for social workers who are not yet licensed or certified at the independent practice level. Social workers at the GS-9 level are working toward completion of prerequisites for licensure or certification. GS-11 Social Worker Promotion to the GS-11 full performance level requires completion of a minimum of 1 year of post-MSW degree clinical experience in the field of health care social work (VA or non-VA experience) and licensure or certification in a state at the independent practice level. OR, In addition to meeting basic requirements, a doctoral degree in social work from a school of social work may be substituted for the required 1 year of professional social work experience in a clinical setting. The work is mostly sedentary. However, occasional standing, walking, bending, and lifting of light items may be necessary. This position does require the operation of a government vehicle and transporting patients, there will be a requirement to pass a driver's physical.
